A care pathway maps the care and treatment patients can expect to receive from us. We
want these to centre around people’s individual needs rather than their diagnosis.
Come along and take a look at the pathways we have developed and tell us what you think.
At the meeting 21 care pathways will be on display for people to look at – care pathways map out the care people may receive depending on their diagnosis and individual care needs, and explain the anticipated care, treatment and medication people are likely to receive. We are keen to hear people’s thoughts and suggestions and ideas for improving them.
